Abstract:Objective To investigate serum nucleotide-bound oligomeric domain-like receptor protein 3(NLRP3)re-lated factors in the evaluation of cerebral thrombosis in patients with acute cerebral infarction(ACI)and its clinical applica-tion value. Methods In total,120 patients with ACI treated from January to December 2022 were selected as the observation group, and 120 healthy physical examinees were selected as the control group.Observation group was treated with mechanical thrombectomy and divided into thrombus group and non-thrombus group according to obtainment of thrombus tissue. The levels of serum NOD-like receptor thermal protein domain associated protein 3(NLRP3),Cysteine aspartic protease 1(Caspase-1),interleukin-1β(IL-1β),interleukin-18(IL-18)levels in the observation group and the control group were compared, and the clinical data of the thrombus group and the non-thrombus group were analyzed. The factors related to cerebral thrombosis in ACI patients were analyzed. The value of serum NLRP3 inflammasome-signaling pathway in the evaluation of ACI cerebral thrombosis and its clinical application was investigated, and the clinical outcome of ACI cerebral thrombosis was analyzed. Results The levels of NLRP3,Caspase-1,IL-1β and IL-18 in the observation group were higher than those in the control group, and higher in the thrombus group than in the non-thrombus group(P<0.01).Multivariate Logistic regression analysis showed that serum NLRP3,Caspase-1,IL-1β and IL-18 were independent risk factors for cerebral thrombosis in ACI patients(P<0.01).The area under the receiver operating characteristic(ROC)curve(AUC)of serum NLRP3,Caspase-1,IL-1βand IL-18 in combination in assessment of cerebral thrombosis was higher than that of single detection, and the threshold range of net benefit of combined evaluation was greater than that of single detection(P<0.05).The proportion of mRS scores of 4-6 points in thrombus group was higher than that in non-thrombus group(P<0.05).Conclusion The serum levels of NLRP3,Caspase-1,IL-1β and IL-18 in ACI patients are elevated, and the combined detection of NLRP3,Caspase-1,IL-18 has a certain evaluation value for cerebral thrombosis, and has a higher clinical net benefit.
