Efficacy of Tofacatib Combined with Leflunomide in the Treatment of Re-fractory Rheumatoid Arthritis

Abstract：Objective To observe the efficacy of Tofacitib and Etanercept combined with Leflunomide in the treat-ment of refractory rheumatoid arthritis(RA).Methods Sixty patients with refractory RA from April 2020 to September 2021 were selected and divided into observation group(n =30)and control group(n =30)according to treatment methods.The observation group was treated with Tofacitib combined with Leflunomide,and the control group was treated with Etanercept combined with Leflunomide.The clinical effect,improvement of physical signs,serum rheumatoid factor(RF),C reactive protein(CRP),erythrocyte deposition rate(ESR),liver and kidney function index,rheumatoid arthritis condition(DAS28)-ESR score,and visual analog scale(VAS)score before and after treatment were compared between the two groups,and the safety was analyzed.Results The total effective rate of the observation group was 86.67％(26/30),which was higher than that of the control group[63.33％(19/30)](P＜0.05).At 12 and 24 weeks after treatment,the number of joint swelling,the number of tender joints and the duration of morning stiffness in the two groups were reduced or shortened,which was more significant in the observation group than in the control group(P＜0.05,P＜0.01).The levels of serum CRP,ESR,RF,DAS28-ESR score and VAS score in the two groups were lower than those at 12 and 24 weeks after treat-ment,and lower in the observation group than in the control group(P＜0.05,P＜0.01).There were no significant differ-ences in aspartate aminotransferase,blood urea,alanine aminotransferase and blood creatinine between and within the two groups before treatment,at 12 weeks and 24 weeks after treatment(P＞0.05).There was no significant difference in the inci-dence of adverse reactions between the two groups(P＞0.05).Conclusion Tofaciib and Etanercept combined with Lefluno-mide respectively have a definite effect in the treatment of refractory RA,but Tofaciib combined with Leflunomide has a better effect,which can improve the clinical symptoms of patients,regulate the expression of inflammatory factors,contribute to the disease control of patients,and have good safety.
