Meta-analysis on prognostic assessment of motor function in Chinese elder ischemic stroke patients

Abstract:Objective To investigate the clinical value of the diffusion tensor imaging ( DTI) in assessing the motor function in the prognosis of elderly ischemic stroke disease in domestic re-search. Methods Retrieving published literature in China National Knowledge Infrastruction ( CNKI) , VIP database ( VIP ) and Wanfang database before December 2015, analyzing with hete-rogeneity test, using a random effects model to merge the effect index value, drawing forest plot, and making inverted funnel analysis, part of the literature making the descriptive analysis. Results A total of 46 documents were retrieved efficiently,and divided into three groups according to the ex-perimental design. For the cohort study group, the change of fractional anisotropy ( FA) values for 12 weeks after the onset of infarction, had a decreasing trend,the change of FA value for 4 weeks was smaller than those for the 1 weeks and 12 weeks;in the quasi-randomized studies in the litera-ture, the average FA of the cerebral infarction in the ipsilateral hind capsule, cerebral peduncle and the corticospinal tract was significantly decreased ( P<0. 05 ) when compared to the contralateral healthy parts;in the cross-sectional studies in the literature, DTI can directly observe the changes in the white matter tracts, and assess the motor function of limbs. Conclusion Four weeks after the onset of ischemic stroke disease, the changes in FA of DTI trends to be stabilized, and it could match better with the limb function assessment tool. The FA value could be used to predict the treat efficacy, but the stability and reliability of the domestic literature still need to be improved.
