Investigation of a dengue fever outbreak at a construction site in Shenzhen

Abstract:Objective To conduct a field epidemiological investigation of a dengue fever outbreak at a construction site in Shenzhen,analyze its epidemiological characteristics and influencing factors,and to summarize the experience.Methods The cases and the mosquito surveillance data were collected,the temporal,spatialand population distributions of cases were described.Retrospective cohort study was used to compare and analyze the differences of the incidence rates among different groups and their causes.The targeted control measures were formulated,and their effects were evaluated.The incidence rates were compared using the Pearson's Chi-square test or Fisher's exact test.Results From September 19 to October 11,2019,a total of 32 cases were reported,with the incidence rate of 11.76%(32/272).The main clinical symptom was fever(100.00%),and all cases were mild.There was no statistically significant difference in the incidence rates between males and females(χ2=0.038,P=0.846),or between on-site construction workers and project managers(χ2=0.746,P=0.446).However,the incidence rate of cutting team 2 among on-site construction workers was significantly higher than that of other teams(RR=3.667,95%CI:1.926-6.982),and the incidence rate of the demolition team was significantly higher than that of other teams(RR=3.173,95%CI:1.426-7.059).After a series of measures,including case isolation,rapid mosquito killing,and breeding site cleanup,the Aedes mosquito density and the number of cases significantly decreased,and the outbreak was effectively controlled after 23 days.Conclusion Due to the complex environment of construction site,it is difficult to control the density of Aedes mosquitoes,and the self-protection awareness of the workers were poor,and therefore construction site are more likely to outbreak dengue fever.Medical institutions should popularize the use of NS1 antigen reagent to promote the early detection and diagnosis of the epidemic.Construction and supervision units should prioritize mosquito control as an essential aspect of project management and regularly carry out mosquito control works.Additionally,attention should be paid to the health monitoring of workers to avoid introducing infectious sources.
