Risk factors for deep vein thrombosis after thrombolysis in patients with acute ischemic stroke and construction of a nomogram model

Abstract：Objective To analyze the risk factors for deep vein thrombosis(DVT)after thrombolysis in patients with acute ischemic stroke(AIS)and construct a nomogram model.Methods A total of 212 AIS patients who received intravenous thrombolysis in our hospital from April 2023 to March 2025 were selected and divided into DVT group(n=47)and non-DVT group(n=165)based on the occurrence of DVT in hospital.The influencing factors of DVT after thrombolysis were evaluated by multivariate logistic regression.A nomogram model for predicting DVT after thrombolysis was constructed,and its predictive value was evaluated by the ROC curve.Results The DVT group had higher proportions of older age,diabetes history,and consciousness disorder,longer time from onset to thrombolysis and bed rest time,and higher NIHSS score,platelet count,LDL-C,and D-dimer level at admission than the non-DVT group(P<0.05).Multivariate analysis showed that older age(OR=1.960,95%CI:1.399～2.746),diabetes history(OR=2.392,95%CI:1.626～3.519),consciousness disorder(OR=2.838,95%CI:1.869～4.308),longer bed rest time(OR=1.774,95%CI:1.314～2.394),and higher D-dimer level(OR=1.863,95%CI:1.351～2.569)were risk factors for DVT after thrombolysis in AIS patients(P<0.05).The consistency index(C-index)of the nomogram model was 0.897(95%CI:0.846～0.948),and the original curve was close to the ideal curve.The area under the curve(AUC)(95%CI)of the nomogram model for predicting DVT after thrombolysis in AIS patients was 0.906(0.855～0.957),with a specificity of 68.42%and a sensitivity of 91.79%.Conclusions Age,history of diabetes,disturbance of consciousness,bed time,and D-dimer are risk factors for DVT in hospital after thrombolysis in AIS patients.The nomogram model based on this conclusion has good prediction performance and clinical application value.
