Predictive value of combined monitoring of intracranial pressure and brain tissue oxygen partial pressure with serum S100B for the prognosis of patients with traumatic brain injury

Abstract:Objective To explore the predictive value of combined monitoring of intracranial pressure(ICP),brain tissue oxygen partial pressure(PbtO2),and serum S100B for the prognosis of patients with traumatic brain injury(TBI).Methods A total of 148 TBI patients from January 2022 to January 2025 were divided into a good prognosis group(n=103)and a poor prognosis group(n=45)based on the prognosis at 3-month follow-up.The levels of ICP,PbtO2,and serum S100B were compared in these two groups.The predictive value of ICP,PbtO2,and S100B for the prognosis of TBI patients was evaluated using the ROC curve.The influencing factors of prognosis in TBI patients were investigated by multivariate logistic stepwise regression.Results The levels of ICP and serum S100B were higher in the poor prognosis group than in the good prognosis group,while PbtO2 was lower in the poor prognosis group than in the good prognosis group(P<0.05).The area under the curve(AUC)for predicting the prognosis of TBI patients by ICP,PbtO2 and S100B was 0.768,0.810 and 0.861 respectively(Z=4.680,4.091,2.893,all P<0.001),which was lower than that of the combined prediction of the three indicators(AUC=0.945).The proportion of patients aged≥60 years,hematoma volume,and the proportion of patients with brain herniation in the poor prognosis group were higher than those in the good prognosis group,but the Glasgow Coma Scale(GCS)score at admission of the poor prognosis group was lower(P<0.05).Low GCS score at admission(OR=0.051,95%CI:0.012~0.218),brain herniation(OR=7.283,95%CI:1.332~39.826),high ICP(OR=11.879,95%CI:2.802~50.356),low PbtO2(OR=0.101,95%CI:0.024~0.413),and elevated S100B level(OR=16.127,95%CI:3.744~69.462)were risk factors for poor prognosis in TBI patients(P<0.05).Conclusions Elevated ICP,decreased PbtO2,and elevated serum S100B level are associated with the increased risk of poor prognosis in TBI patients,and the combined prediction of the three indicators has a higher value in predicting the prognosis of such patients.
