Efficacy and safety of different doses of butylphthalide in the treatment of acute ischemic stroke

Abstract:Objective To investigate the efficacy and safety of different doses of butylphthalide in the treatment of acute ischemic stroke(AIS).Methods A total of 86 AIS patients admitted to our hospital from January 2019 to November 2020 were selected and randomly assigned into an observation group and a control group,with 43 patients in each group.Both groups received conventional treatment.On this basis,the control group was treated with 100 ml of butylphthalide injection once daily,while the observation group received 100 mL twice daily,for a total of 14 days.The National Institute of Health Stroke Scale(NIHSS),Activity of Daily Living Scale(ADL),and Fugl-Meyer Assessment(FMA)scores were evaluated before and after treatment.Serum levels of C-reactive protein(CRP),protein kinase C(PKC),pentameter protein 3(PTX3),tumor necrosis factor-α(TNF-α),osteoprotegerin(OPG),and soluble tumor necrosis factor-related apoptosis-inducing ligand(sTRAIL)were detected.Adverse reactions during treatment were recorded.Results After 14 days of treatment,the total effective rate in the observation group was significantly higher than that in the control group(93.02%vs 74.42%,P<0.05)and the NIHSS score was lower in the observation group compared to the control group(7.23±1.18 vs 9.47±1.36,P<0.05),while ADL and FMA scores were significantly higher in the observation group(P<0.05).The levels of CRP,PKC,PTX3,sTRAIL,OPG,and TNF-αdecreased in both groups,with the observation group showing significantly lower levels than the control group(all P<0.05).At the 3-month follow-up,the observation group continued to show superior neurological and functional recovery(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Combined treatment with a high dose of butylphthalide(100 ml,twice daily)on the basis of conventional therapy can more effectively improve neurological function,daily living ability,and motor function in AIS patients.It significantly reduces the levels of inflammatory and apoptosis-related markers without increasing adverse reactions,demonstrating good safety.
