Postoperative fatigue in elderly patients with hypertensive intracerebral hemorrhage and its related factors

Abstract:Objective To investigate the status of postoperative fatigue in elderly patients with hypertensive intracerebral hemorrhage and analyze its related influencing factors.Methods A cross-sectional study was consecutively conducted on 100 elderly patients with hypertensive intracerebral hemorrhage who underwent surgical treatment in Qilu Hospital(Qingdao)from January 2022 to January 2025.Their general and clinical data were collected,and the Revised Piper Fatigue Scale(PFS-R)was used to assess their fatigue status.The correlation between patients'fatigue status and various factors was analyzed,and univariate analysis and multiple linear regression analysis were used to identify independent influencing factors for postoperative fatigue.Results The total PFS-R score of the 100 elderly patients with hypertensive intracerebral hemorrhage was(6.15±1.68),indicating a moderate level of overall fatigue,with the sensory dimension scoring the highest.Univariate analysis showed that there were statistically significant differences in fatigue scores among patients with different surgical methods,intraoperative blood loss,preoperative NIHSS score,postoperative complications,C-reactive protein(CRP),hemoglobin(Hb),hypoproteinemia,sleep disorders and pain degree(all P<0.05).Pearson correlation analysis showed that intraoperative blood loss,preoperative NIHSS score,postoperative CRP level,pain VAS score,Lac level,SDBG,and COR level were significantly and positively correlated with the total postoperative PFS-R score.In contrast,postoperative Hb level,serum Alb level,grip strength and average daily step count were significantly and negatively correlated with the total PFS-R score(all P<0.05).Multiple linear regression analysis further indicated that craniotomy(β=0.199,P=0.040),high NIHSS score(β=0.307,P<0.001),high CRP(β=0.255,P=0.009),hypoalbuminemia(β=0.224,P=0.013),sleep disorders(β=0.261,P=0.004),and moderate to severe pain(β=0.235,P=0.026)were independent risk factors for postoperative fatigue.Conclusions Moderate fatigue is prevalent among elderly patients with hypertensive intracerebral hemorrhage after surgery,with the sensory dimensions being the most prominent.Neurological deficits,inflammatory response,nutritional status,sleep quality,and pain severity are key factors influencing postoperative fatigue.To alleviate fatigue and promote recovery,clinical attention should be given to early identification and comprehensive interventions,such as optimizing surgical approaches,enhancing nutritional support,controlling inflammation and pain,and improving sleep quality.
