Peripheral blood levels of MMP-9,25(OH)D,NLR and hemorrhagic transformation in patients with ischemic stroke after interventional thrombectomy

Abstract:Objective To analyze the relationship between peripheral blood matrix metalloproteinase-9(MMP-9),25-hydroxyvitamin D[25(OH)D],neutrophil-to-lymphocyte ratio(NLR)and hemorrhagic transformation(HT)in patients with ischemic stroke after interventional thrombectomy.Methods The clinical data of 116 patients with acute ischemic stroke(AIS)admitted to Xingtai Central Hospital from February 2023 to January 2025 were retrospectively collected.Patients were divided into the HT group(n=34)and the non-HT group(n=82)based on the occurrence of HT after thrombectomy.Clinical data were compared between the two groups,and the influencing factors of HT were analyzed using multivariate logistic regression.Receiver operating characteristic(ROC)curves were plotted,and the areas under the curve(AUCs)were used to evaluate the predictive efficacy of these factors for HT.Results The HT group showed an increase in preoperative NIHSS score,the proportion of patients with more than two times of thrombectomy,the levels of D-dimer,MMP-9,and NLR,and a decrease in 25(OH)D level compared to the non-HT group(P<0.05).Logistic regression analysis demonstrated that elevated levels of MMP-9(OR:2.215,95%CI:1.190-3.797)and NLR(OR:2.651,95%CI:1.299-5.411)were independent risk factors for HT following endovascular thrombectomy in patients with AIS.Conversely,increased level of 25(OH)D(OR:0.438,95%CI:0.236-0.812)was identified as an independent protective factor(all P<0.05).ROC curve analysis showed that the AUC values of MMP-9,25(OH)D,and NLR for predicting HT were 0.745,0.766,and 0.772 respectively.The combined AUC of these factors was 0.926(95%CI:0.862-0.966),with a sensitivity of 88.24%and specificity of 87.80%.The AUC was significantly greater than that of each indicator(ZMMP-9=3.376,Z25(OH)D=3.377,ZNLR=3.436,all P<0.05).Conclusions Peripheral blood MMP-9,25(OH)D,and NLR are closely related to HT risk in patients with AIS after interventional thrombectomy.These markers can serve as predictors for HT,and their combined detection can improve the predictive efficacy.
