Abstract:Cerebral hemorrhage is a disease with extremely high incidence and mortality rate in China, and cerebral edema is its critical pathological process after cerebral hemorrhage, which is quite challenging to manage.Proper management of cerebral edema can significantly reduce mortality.Currently, the treatment options for post-hemorrhagic cerebral edema include hyperosmolar therapy, cerebrospinal fluid drainage, steroid treatment, therapeutic hypothermia, deep sedation with anesthetics, and decompressive craniectomy. These treatments mainly aim to prevent the increase in intracranial pressure and brain herniation in the short term, without addressing the root cause for cerebral edema. Therefore, there is an urgent need to develop novel drugs targeting specific molecular pathways for treating cerebral edema.Utilizing molecular pathophysiology to prevent or reduce the formation of edema is a feature of the next generation anti-edema therapy. This article reviews the latest research on the mechanisms of post-hemorrhagic cerebral edema, and provides ideas for the exploration of new treatment methods.
