Abstract:Cranio-cerebral gunshot injury (CGI) has a high mortality rate, is insidious, and is prone to being missed. This article reports a case of missed CGI found during secondary triage of mass casualties in a peacekeeping mission area. The patient, a 30-year-old male black individual, was a local employee of the United Nations who was injured in a car accident caused by armed militants. The initial triage diagnosed it as cranio-cerebral trauma caused by the car accident, and the casualty was classified as Category C; during secondary triage, an occult wound deep into the bone above the left eyebrow was found, accompanied by symptoms such as fixed left eyeball and aggravated consciousness disorder, which highly suspected CGI, so the casualty classification was adjusted to Category A. After transfer, CGI was confirmed, and the patient was later air-evacuated to a tertiary hospital for treatment.
