Individualized surgical treatment strategies for hypertensive thalamic hemorrhage

Abstract:Objective To evaluate the individualized surgical decision-making,selection of surgical methods,perioperative management,and prognostic outcomes for hypertensive thalamic hemorrhage.Methods Clinical data of 65 patients with hypertensive thalamic hemorrhage treated between January 2015 and December 2023 were retrospectively analyzed.Individualized surgical treatment was implemented based on hematoma volume,morphology,clinical manifestations,and imaging characteristics.Results At 3 months postoperatively,Glasgow Outcome Scale(GOS)assessment showed good recovery in 22 cases(33.8%),moderate disability in 11(16.9%),severe disability in 20(30.8%),vegetative state in 9(13.8%),and death in 3(4.6%).Conclusions Multidimensional individualized surgical decision-making incorporating hematoma characteristics,clinical manifestations,imaging features,and systemic conditions can significantly reduce mortality and improve functional outcomes in hypertensive thalamic hemorrhage,when combined with minimally invasive techniques(stereotactic aspiration,neuroendoscopic evacuation,EVD)and refined perioperative management.
