Comparison of the effects of neuroendoscopic minimally invasive surgery and small bone window craniotomy for supratentorial hemorrhage≤50 mL

Abstract:Objective To compare the efficacy of neuroendoscopic minimally invasive surgery and small bone window craniotomy for supratentorial hemorrhage≤50 mL.Methods A retrospective selection was made of 136 patients with supratentorial intracerebral hemorrhage with a hematoma volume of≤50 mL from June 2022 to August 2024.They were divided into the neuroendoscopy group(n=75)and the craniotomy group(n=61)according to the different surgical methods in the data.The neuroendoscopy group underwent neuroendoscopic minimally invasive surgery,while the craniotomy group underwent small bone window craniotomy.The surgical-related indicators,treatment effects(GOS),neurological functions(NFDS),quality of life(ADL,WHOQOL-BREF),and postoperative complications were analyzed.Results The operation time,catheterization time,and hospital stay of the neuroendoscopy group were significantly shorter,and the hematoma clearance rate was higher compared with craniotomy group(P<0.05).The effective rate of the neuroendoscopy group was 84.00%(63/75),higher than that of the craniotomy group(65.57%,40/61)(P<0.05).For the comparison of NFDS scores between the two groups before operation,1 month after operation and 3 months after operation,the time effect,group effect,and the interaction effect between time and group were all statistically significant(P<0.001);After 1,3 months of operation,the NFDS scores of both groups were decreased,and the NFDS score in neuroendoscopy group was lower(P<0.05).For the comparison of ADL and WHOQOL-BREF scores between the two groups before operation and 3 months after operation,the time effect,group effect,and the interaction effect between time and group were all statistically significant(P<0.001);After 3 months of operation,the ADL scores and WHOQOL-BREF scores of both groups were higher than those before the operation,and the ADL scores and WHOQOL-BREF scores in neuroendoscopy group were higher(P<0.05).The total rate of postoperative complications in neuroendoscopy group(8.00%)was lower compared with craniotomy group(27.87%)(χ2=9.451,P=0.002).Conclusions Neuroendoscopic minimally invasive surgery has definite efficacy for supratentorial hemorrhage≤50 mL compared with small bone window craniotomy,with the advantages of rapidity,fewer complications,and faster recovery.It can better improve the neurological deficits and quality of life of the patients.
