Abstract:This article reports four patients with intracranial aneurysms who were receiving anticoagulant therapy preoperatively and experienced oral hemorrhage after endovascular treatment. The bleeding was considered to be caused by mucosal injury due to tracheal intubation. After local compression, electrocoagulation hemostasis, or hormone nebulization by the department of otolaryngology, and adjustment of antiplatelet drugs (discontinuation of tirofiban, temporary single-drug maintenance, or delayed resumption of dual antiplatelet therapy), the bleeding was controlled in all cases. Antiplatelet therapy during the perioperative period of intracranial aneurysm endovascular treatment increases the risk of oral hemorrhage, and standardization of tracheal intubation is key to prevention. If bleeding occurs postoperatively, timely hemostasis and individualized adjustment of antiplatelet therapy are required.
