Neuroendoscopic"one-and-a-half"nasal sphenoidal approach for pituitary adenomas:a report of 60 cases and its effect on olfactory function

Abstract:Objective To explore the efficacy of the"one-and-a-half"endoscopic transsphenoidal approach in the treatment of pituitary adenomas and its impact on olfactory function.Methods Sixty patients with pituitary adenomas admitted to the Department of Neurosurgery,Union Shenzhen Hospital,from March 2021 to February 2024 were selected as the research subjects and randomly divided into the study group and the control group,with 30 cases in each group.The study group underwent the"one-and-a-half"endoscopic transsphenoidal approach,while the control group underwent the microscopic transsphenoidal approach.The surgical indicators(operation time,intraoperative blood loss,hospital stay),postoperative olfactory function,and the incidence of adverse reactions were compared between the two groups.Results The operation time,intraoperative blood loss,and hospital stay of the study group were significantly lower than those of the control group(P<0.05).The olfactory recognition threshold of the study group was significantly lower than that of the control group(P<0.05).The incidence of complications in the study group was significantly lower than that in the control group(P<0.05).Conclusions Compared with the microscopic transsphenoidal approach,the"one-and-a-half"endoscopic transsphenoidal approach for the treatment of pituitary adenomas has a significant clinical effect,can effectively reduce the damage to patients,and promote the recovery of olfactory function,which is worthy of clinical application.
