MRI images of diffuse hemispheric glioma with H3 G34 mutation

Abstract:Objective To investigate the MRI images,pathological and prognostic changes of diffuse hemispheric glioma(G34-DHG)with H3G34 mutation and to improve the understanding of this tumor and preoperative diagnosis.Methods Retrospective analysis of the clinical manifestations,MRI images,pathological changes,and prognostic features of 12 cases of diffuse hemispheric gliomas with H3G34 mutation was confirmed by clinicopathological study.Results Lesions of all the 12 patients were located in the same cerebral hemisphere,and were common in the frontal,temporal,parietal,and basal ganglia regions,and were rare in the hippocampus,occipital lobe,etc.They grew diffusely and generally involved several cerebral lobes.The lesions generally showed mixed equal/slightly long T1WI and slightly long/long T2WI signals.Three cases had obvious peritumoral edema,while the other 9 cases had no or slight peritumoral edema.Enhanced scanning showed various enhancement patterns,with 8 cases having varying degrees of enhancement and 4 cases having no enhancement.Seven cases showed varying degrees of cystic changes and necrosis,while 3 cases showed cystic-solid changes,with more cystic components than solid components.Four cases had a small amount of intratumoral bleeding.One case had a T2-FLAIR mismatch sign,and the case had an enhanced T1-ASL mismatch phenomenon.Conclusion G34-DHG has specific locations and MRI characteristics,including the diverse contrast-enhancement patterns,no-or slightly peritumoral edema.The lesions are all located in the same cerebral hemisphere.The definite diagnosis depends on pathological and molecular detection.
