Risk factors for postoperative complications of radial artery cerebral aneurysm intervention and construction of a nomogram prediction model

Abstract:Objective To explore the risk factors for postoperative complications of radial artery cerebral aneurysm intervention and construct a nomogram prediction model.Methods A total of 204 patients with radial artery cerebral aneurysm treated in the First Affiliated Hospital of Henan University of Chinese Medicine from February 2021 to February 2025 were selected as the subjects of this study.They were divided into a complications group(n=53)and a non-complications group(n=151)based on the occurrence of postoperative complications.Univariate and multivariate logistic stepwise regression analysis were used to investigate the influencing factors of postoperative complications in patients undergoing radial artery cerebral aneurysm intervention.A nomogram model of postoperative complications in patients undergoing radial artery cerebral aneurysm intervention surgery was constructed.Receiver operating characteristic(ROC)curves were used to evaluate the predictive value of the nomogram model for postoperative complications in patients undergoing radial artery cerebral aneurysm intervention.Results The age,the proportion of diabetes history,and the proportion of radial artery punctures≥2 times in the complications group were all greater than those in the non-complications group.The radial artery diameter,the amount of heparin used during the operation were smaller than those in the non-complications group.The time of compression hemostasis after the operation was longer than that in the non-complication group.The levels of C-reactive protein and low-density lipoprotein cholesterol(LDL-C)were higher than those in the non-complication group(P<0.05).Multivariate analysis showed that age(OR=2.040,95%CI:1.456~2.858),history of diabetes(OR=1.912,95%CI:1.394~2.621),number of radial artery puncture≥2(OR=2.524,95%CI:1.669~3.817),and high C-reactive protein levels(OR=2.328,95%CI:1.582~3.425)were risk factors for postoperative complications of patients with radial artery cerebral aneurysm intervention(P<0.05).The consistency index(C-index)of the nomogram model was 0.906(95%CI:0.854~0.958);the area under the curve(AUC)(95%CI)of the nomogram model for predicting postoperative complications in patients with radial artery cerebral aneurysm intervention is 0.892(0.840-0.944),with specificity 68.36%and sensitivity 91.98%.Conclusions The influencing factors for postoperative complications in patients with radial artery cerebral aneurysm undergoing interventional surgery include age,history of diabetes,number of radial artery punctures,and C-reactive protein.The nomogram model based on the above factors has high predictive value.
