Transcranial ultrasound and CT in ventricular measurement of patients undergoing cranioplasty during the perioperative period

Abstract:Objective To explore the practical value of bedside transcranial ultrasound for perioperative ventriculometry in patients undergoing cranioplasty with poly-ether-ether-ketone(PEEK)repair materials.Methods The clinical data of patients undergoing cranioplasty with PEEK materials admitted to the Department of Neurosurgery,Huadong Hospital Affiliated to Fudan University from June 2021 to June 2023 were prospectively analyzed.Lateral ventricles frontal horns(IFH)and third ventricle diameters(TV)were measured using bedside transcranial ultrasound,and the corresponding indexes were compared and analysed with the parameters of the CT scan results at the same time point.Results IFH was not significantly different from the measurements of trans-PEEK ultrasound(40.62±9.15 mm)and cranial CT(40.79±9.02 mm)(P=0.156),with a correlation coefficient of 0.987.TV was not significantly different from the measurements of trans-PEEK ultrasound(40.62±9.15 mm)and cranial CT(40.79±9.02 mm)(P=0.751),with a correlation coefficient of 0.960.In patients with repair combined with shunt,postoperative IFH(44.18±9.79 mm)was significantly smaller(47.07±9.38 mm)compared with preoperative(P=0.005),whereas in patients with repair alone,postoperative IFH(38.52±7.86 mm)was significantly larger(35.80±8.26 mm)compared with preoperative(P=0.005).Conclusions Ultrasound images of perioperative ventricular parameters measured in patients via PEEK are of high quality,easy to operate,safe and reliable,and allow dynamic long-term monitoring of ventricular size and cranial cavity contents to guide clinical decision-making.
