Clinical value of pre-surgical vascular interventions for head and neck masses

Abstract:Objective To explore the clinical application value of preoperative vascular intervention therapy for embolization and internal carotid artery balloon occlusion test(BOT)in the surgical treatment of head and neck tumors.Methods A retrospective study was conducted on the clinical data of 11 patients with head and neck tumors who were admitted to the Interventional Pain Department of the 940th Hospital of the Joint Logistics Support Force from February 2019 to February 2022.Prior to surgery,the modified Seldinger technique was used to perform bilateral internal and external carotid artery and vertebral artery angiography through the femoral artery puncture approach in order.Following the imaging results,the blood supply arteries of the internal and external cervical systems of the tumor were embolized using super selection,and surgical resection was performed within 24 hours after embolization.Results Of the 11 patients,6 were supplied with the external carotid artery branch on the affected side,3 were supplied with the maxillary artery branch on the affected side,and 1 was supplied with the arterial branch on the affected side and the other 1 with the superficial temporal artery branch on the affected side.Surgical resection was performed on the 11 patients within 1 day after embolization,with minimal intraoperative bleeding and a relatively clear boundary between the tumor and surrounding tissues,which benefited the surgical separation.The average amount of bleeding during surgery was approximately 230 mL,and the average duration of surgery was 2.72 hours.Conclusion Preoperative super selective vascular intervention for head and neck tumors can significantly reduce intraoperative bleeding and shorten surgical time.Preoperative vascular intervention for head and neck tumors is a minimally invasive treatment method.
