Emergency surgical treatment strategy for ruptured middle cerebral artery aneurysm

Abstract:Objective To summarize the efficacy of emergency craniotomy and microscopic clipping for ruptured middle cerebral artery aneurysm(MCA).Methods A retrospective analysis was done on the clinical data of 40 cases of ruptured middle cerebral artery aneurysms treated with emergency craniotomy and clipping in our department from August 2020 to April 2024.Results Follow-up head CTA two weeks post-surgery indicated complete aneurysm clipping.Upon discharge,Glasgow Outcome Scale(GOS)scores were as follows:4-5 points in 27 cases,3 points in 7 cases,2 points in 3 cases,and 1 point in 3 cases.The favorable outcome rate was 67.5%(27/40),and the mortality rate was 7.5%(3/40).Postoperative follow-up ranged from 3 to 30 months.Conclusion Emergency microscopic clipping for ruptured middle cerebral artery aneurysms can yield satisfactory outcomes in patients.Factors such as the preoperative Glasgow Coma Scale(GCS)score,aneurysm morphology,the presence of concomitant hematoma,treatment strategy,and the surgical skills of the operator are all associated with the patient's prognosis.
