Effect of neuroendoscopic transsphenoidal approach resection on pituitary adenoma and its influence on serum GH and PRL levels

Abstract:Objective To analyze the effect of neuroendoscopic transsphenoidal approach resection on pituitary adenoma and its influence on serum growth hormone(GH)and prolactin(PRL)levels.Methods The clinical data of patients from August 2022 to October 2024 with pituitary adenoma in the First Affiliated Hospital of Hebei North University were retrospectively analyzed.According to the differences in surgical methods,the patients were divided into endoscopic group(neuroendoscopic transsphenoidal approach pituitary adenoma resection)and microscopic group(microscopic transsphenoidal approach pituitary adenoma resection).The baseline data of the two groups were balanced by propensity score matching,and 1:1 ratio matching was used to obtain 68 patients in each group with comparable baseline data.The perioperative indicators(intraoperative blood loss,surgical time,hospitalization time),tumor resection effect and incidence rates of postoperative complications and endocrine indicators(serum GH,PRL)before surgery and at 3 months after surgery were compared between the two groups.Results The intraoperative blood loss,surgical time and hospitalization time in endoscopic group were less or shorter than those in microscopic group(P<0.05).There was no significant difference in tumor resection effect between the two groups(P>0.05).The levels of serum GH and PRL in the two groups were decreased at 3 months after surgery(P<0.05),and the levels in endoscopic group were lower than those in microscopic group(P<0.05).The incidence rate of transient diabetes insipidus and total incidence rate of postoperative complications in endoscopic group were lower than those in microscopic group(P<0.05).Conclusion In the treatment of pituitary adenoma,microscopic and neuroendoscopic transsphenoidal approach resection methods have a similar tumor resection effect,but neuroendoscopic transsphenoidal approach has milder trauma,better endocrine hormone control and higher safety.
