Serum miR-130b and miR-148b-3p levels and cerebral vasospasm after clipping surgery in patients with intracranial aneurysms

Abstract:Objective To explore the relationship between serum miR-130b and miR-148b-3p levels and cerebral vasospasm after clipping surgery in patients with intracranial aneurysms(IA).Methods A total of 152 IA patients admitted to the First Hospital Affiliated to Northwest University from January 2021 to December 2024 were included in this study.They were divided into the vasospasm group(n=37)and the non-vasospasm group(n=115)based on the occurrence of cerebral vasospasm clipping.The predictive value of serum miR-130b and miR-148b-3p for cerebral vasospasm after clipping in IA patients was evaluated using the receiver operating characteristic(ROC)curve.The influencing factors of cerebral vasospasm after clipping in IA patients were evaluated by multivariate Logistic stepwise regression.Results The levels of serum miR-130b and miR-148b-3p in the vasospasm group were higher than those in the non-vasospasm group(P<0.05).The area under the curve(AUC)(95%CI)for predicting cerebral vasospasm after clipping in IA patients by serum miR-130b,miR-148b-3p,and their combination was 0.847(0.794-0.900),0.769(0.716-0.822),and 0.902(0.849-0.955)respectively.The age of the spastic group was≥60 years old,with a history of hypertension,Hunt Hess grading(Ⅲ-Ⅳ),the proportion of aneurysms with a diameter≥1 cm,and intraoperative blood loss were all higher than those in the non spastic group.The proportion of preoperative use of calcium ion antagonists was lower than that in the non-spastic group,and the white blood cell count level was higher than that in the non-spastic group(P<0.05).Age≥60 years(OR=1.972,95%CI:1.410-2.757),Hunt-Hess grade(Ⅲ-Ⅳ)(OR=2.812,95%CI:1.824-4.337),aneurysm diameter≥1 cm(OR=2.259,95%CI:1.548-3.298),high expression of miR-130b(OR=2.654,95%CI:1.731-4.069),and high expression of miR-148b-3p(OR=2.416,95%CI:1.613-3.617)were risk factors for cerebral vasospasm after clipping in IA patients(P<0.05).Conclusion Elevated levels of serum miR-130b and miR-148b-3p are associated with cerebral vasospasm after clipping in IA patients and can be used as potential predictive markers.
