Correlation between RLS and asymptomatic cerebral ischemic injury in migraine patients:an analysis based on c-TCD and MRI

Abstract:Objective To investigate the correlation between right-to-left shunt(RLS)and asymptomatic cerebral ischemic injury in migraine patients based on contrast-transcranial Doppler(c-TCD)and magnetic resonance imaging(MRI).Methods A total of 114 migraine patients who underwent c-TCD and MRI examinations at Ankang City Central Hospital from March 2021 to December 2024 were selected,and the examination results were analyzed.The incidence rates of silent brain infarction(SBI)and white matter hyperintensity(WMH)in migraine patients with different RLS grades were compared.Spearman's rank correlation was used to discuss the correlation of RLS,SBI,and WMH in migraine patients.Multivariate logistic regression was used to identify the risk factors for asymptomatic cerebral ischemic injury in migraine patients.Results c-TCD found 48 cases of RLS grade 0,31 cases of RLS grade Ⅰ,20 cases of RLS grade Ⅱ,and 15 cases of RLS grade Ⅲ.There were 27 cases of permanent RLS and 39 cases of latent RLS.MRI detected 5 cases of silent brain infarction(SBI)and 42 cases of white matter hyperintensity(WMH).The incidence rates of SBI and WMH in migraine patients with RLS grade Ⅲ were significantly higher than those in patients with RLS grade 0(P<0.05).Spearman's rank correlation analysis found that RLS grade was positively correlated with the incidence rates of SBI and WMH(r=0.257,P=0.006;r=0.244,P=0.009).Logistic regression analysis found that RLS grade Ⅲ was a risk factor for asymptomatic cerebral ischemic injury in migraine patients(OR=3.086,95%CI=1.280-7.441,P<0.05).Conclusion RLS is closely related to asymptomatic cerebral ischemic injury in migraine patients.Large RLS can increase the risks of SBI or WMH in migraine patients.And c-TCD can be used for screening and early prevention of migraine.
