Risk of vascular interventional embolization combined with lumbar puncture cerebrospinal fluid replacement in preventing cerebral vasospasm in patients with cerebral aneurysm rupture

Abstract:Objective To analyze the effect of vascular interventional embolization combined with lumbar puncture cerebrospinal fluid replacement on preventing the risk of cerebral vasospasm(CV)in patients with cerebral aneurysm rupture.Methods The clinical data of 188 patients with cerebral aneurysm rupture who received endovascular interventional embolization in the Second Hospital,Lanzhou University from January 2022 to December 2024 were collected by retrospective analysis.Eighty-six patients who did not undergo lumbar puncture cerebrospinal fluid replacement after surgery were selected as control group,and the other 102 patients who received lumbar puncture cerebrospinal fluid replacement after surgery were included in the observation group.The occurrence of CV,cerebrovascular hemodynamics,inflammatory factors and prognosis were counted and compared.Results At 3 d and 7 d after surgery,the middle cerebral artery mean blood flow velocity and Lindegaard ratio in observation group were lower than those in control group(P<0.05).The levels of serum soluble CD40 ligand(sCD40L),soluble intercellular adhesion molecule-1(sICAM-1),interleukin-23(IL-23)and IL-17 were lower in observation group than those in control group(P<0.05).At 30 days of follow-up after surgery,the good prognosis rate in observation group was higher than that in control group(P<0.05).The incidence rates of postoperative cerebral vasospasm and hydrocephalus in observation group were lower than those in control group(P<0.05).Conclusion Vascular interventional embolization combined with lumbar puncture cerebrospinal fluid replacement can effectively reduce the risk of CV in patients with cerebral aneurysm rupture,improve the prognosis,and relieve the inflammatory response.
