Involvement of immune inflammation in coagulation dysfunction under conditions of traumatic brain injury combined with hypothermia:a hypothesis

Abstract:Traumatic brain injury combined with seawater immersed hypothermia(TBI-SIH)has the compound nature of two diseases concurrently,so the pathophysiological mechanism is extremely complex,and there is no consensus on the treatment plan.Comprehensively analysing the results of the previous research of our group and the results reported in the literature,this paper puts forward a new hypothesis:after traumatic brain injury combined with hypothermia,a special crosstalk mechanism occurs between the immune system and the coagulation system,which is manifested in the following way:at the initial stage the inflammatory response leads to a reduction in thrombin generation;however,with the gradual lowering of the core temperature,the inflammatory response is suppressed,the coagulation function is hyperactivated,and the body manifests a hypercoagulable state;and later on,due to the hypothermia,platelet deficiency,reduced thrombin activity,the inflammatory response is aggravated again,and the organism shows a hypocoagulable state,and the interaction of the two systems leads to a composite state of hypocoagulability and hypercoagulability that accelerates the process of death.This hypothesis provides new ideas for the exploration of the mechanism and clinical treatment of TBI-SIH,and may be of great significance to slow down the progression of the two pathologies and block the association between the immune system and the coagulation system to alleviate the deterioration of the condition of the injured.
