One Case of Motor Aphasia After Stent-Assisted Coil Embolization of a Middle Cerebral Artery Aneurysm Using Atlas Stents
Abstract:In the endovascular treatment of wide-necked aneurysms at the bifurcation of the middle cerebral artery (MCA), maintaining the patency of the parent artery and avoiding severe perforator events or distal vessel occlusion remain significant challenges. This article reports a case of a wide-necked aneurysm at the MCA bifurcation treated with two Atlas stents in a 'T' configuration to assist coil embolization. Immediate postoperative angiography showed patency of the parent artery and partial embolization of the aneurysm. However, on the second postoperative day, the patient developed severe distal vessel occlusion leading to motor aphasia. Although some rescue measures were taken promptly, the patient still had residual neurological deficits at discharge. This case provides some clinical experience for future endovascular interventions in wide-necked aneurysms at the MCA bifurcation.
