Occipital artery compression-assisted Onyx-18 endovascular treatment of tentorial dural arteriovenous fistulas

Abstract:Tentorial dural arteriovenous fistula (TDAVF) is an abnormal arteriovenous shunt located in the tentorial and its accessory structures. Because of the deep location of the lesion and the complex feeding arteries and draining veins,the treatment of TDAVF is difficult. Trans arterial embolization with Onyx-18 is currently the preferred treatment. However,the poor diffusion of Onyx-18 in fistulas is a crucial reason for incomplete treatment and residual or recurrence in some patients. Surface compression can block the occipital artery for its superficial position. Few surgeons mentioned extracorporeal compression of the occipital artery to assist Onyx-18 diffusion in TDAVF. 2 cases of TDAVF are reported in which occipital artery compression-assisted ONYX-18 endovascular treatment was applied. The MRI of brain,cerebral angiography,surgical procedures,and clinical results are described,with the literature about treatment of TDAVF discussed and reviewed. These 2 patients with TDAVF were completely cured with good recovery,and the discomfort symptoms disappeared with no surgery-related complications occurred. Occipital artery compression-assisted Onyx-18 endovascular treatment can improve the embolization effect of TDAVF patients whose feeding artery includes occipital artery.
