Surgical treatment of pituitary adrenocorticotropin-secreting adenoma in childhood and adolescence

Abstract:Objective The study aims to investigate the clinical characteristics and treatment of Cushing's disease of childhood and adolescence.Methods In the study the data of 50 cases of pituitary adrenocorticotropinsecreting adenomas of childhood and adolescence,admitted to Peking Union Medical College Hospital from December 1987 to December 2014,were analyzed retrospectively,including the clinical manifestations,hormone secretions,images,pathological types,surgical complications and follow-up results.Results A total of 21 (42.0%) males and 29 females (58.0%) were included,whose mean age was 15.5 years old (ranged from 10 to 18).Microadenora accounted for 80.0% of the total patients,while macroadenoma accounted for 20.0%.A total of 8.0% of adenomas in our study was invasive.Transsphenoidal approach surgery was commonly used for all the cases in our study,2 of which used intm-opemtive navigation assistance.Total resection was achieved in 96.0% of patients,and the rest achieved the subtotal resection.Forty-four (80.0%) patients got full-term follow-up until 1 year after surgery.Conclusion Transsphenoidal approach surgery is the most commonly used and first line surgical therapy.The recurrence rate is about 15.0%,and the patients need regular examination for pituitary hormones and imaging examination study to confirm the recurrence.
