A case report of Tropheryma whipplei pneumonia in an infant and literature review

Abstract:Whipple's disease is a systemic infectious disease caused by the Gram-positive bacterium Tropheryma whipplei(TW).This article analyzed the clinical characteristics,auxiliary examinations,diagnosis and treatment process,and prognosis of a 5-month-old male infant with TW pneumonia,and reviewed relevant literature to improve pediatric clinicians' understanding of this disease.The infant presented with chronic cough.Tropheryma whipplei was detected by metagenomic next-generation sequencing of bronchoalveolar lavage fluid(BALF-mNGS).The patient improved after treatment with penicillin combined with compound sulfamethoxazole and achieved a favorable prognosis during regular follow-up.TW pulmonary infection lacks specific clinical manifestations and is difficult to detect by routine examinations.Early application of BALF-mNGS is recommended for pathogen identification,so as to conduct targeted anti-infection treatment timely,improve survival rate and reduce recurrence rate.
