Effect and safety of high dose topiramate pulse therapy on epilepsy in children

Abstract:Objective To explore the clinical effect and safety of high dose topiramate pulse therapy on epilepsy in children.Methods Sixty-three children with epilepsy admitted to Children's Hospital of Hebei Province from June 2015 to June 2017 were randomly divided into control group (31 cases) and observation group (32 cases).The control group was given topiramate orally at an initial dose of 1 mg/(kg · d),which gradually increased to 6 mg/(kg · d) if there was no serious adverse reaction.The observation group was given topiramate at an initial dose of 10 mg/(kg · d) once daily for 2 days,followed by 10 mg/(kg · d) twice daily for 3 days;the dosage gradually reduced to 7-8 mg/(kg · d).Both groups were treated for 3 months and followed for 6 months.Hospitalization time,decrease rate of epileptic seizure,the curative effect,discharge index and adverse reactions were analyzed.Results There was no significant difference of hospitalization time between groups(P =0.136).The decrease rate of epileptic seizure in observation group was significantly higher than that in control group[(75 ±8) % vs (61 ± 7)%] (P =0.026).The total effective rate in observation group was significantly higher than that in control group[93.8% (30/32) vs 77.4% (24/31)] (P =0.016).After treatment,the discharge index in observation group was significantly lower than that in control group (P =0.038).There was no significant difference of the incidence of adverse reactions between groups (P > 0.05).Conclusion High dose topiramate pulse therapy in children with epilepsy can effectively reduce abnormal brain discharge and:epileptic seizure;the adverse reactions are tolerable.
