Effect of special management of antibiotic prophylaxis in class Ⅰ incision cardiac surgery

Abstract:Objective To explore the effect of special management of antibiotic prophylaxis on the utilization of antibacterials and the prevalence of postoperative infection in patients with class Ⅰ incision cardiac surgery.Methods Defined daily doses(DDDs),anti-microbial use density(AUD),total consumption,commonly used agents of antibacterials and postoperative infection rate of patients with class Ⅰ incision cardiac surgery before (in December 2016 and November 2017) and after(in December 2017) antibiotic prophylaxis management in the Ninth Department of Cardiac Surgery,Beijing Anzhen Hospital,Capital Medical University were retrospectively analyzed.Results In December 2017,DDDs decreased 43.5% (762/1 750) compared to that in December 2016,it decreased 45.9% (839/1 827) compared to that in November 2017;AUD decreased 30.4% (27.83/91.63) compared to that in December 2016 and it decreased 37.3% (37.88/101.68) compared to that in November 2017;the total consumption of antibacterials decreased 54.4% (23.96/44.06) compared to that in December 2016 and it decreased 37.7% (12.15/32.25) compared to that in November 2017.Postoperative infection rate showed no significant difference before(between 1 st December 2016 and 15th January 2017) and after(between 1 st December 2017 and 15th January 2018) antibacterial management [7.5% (10/133) vs 5.1% (7/138)] (P > 0.05).Conclusion Special management of antibiotic prophylaxis can promote reasonable antibiotic use in cardiac surgery and decrease overuse of antimicrobials without increasing postoperative infection rate.
