Clinical effect of intra-aortic balloon pump on severe acute left cardiac insufficiency after mitral valve surgery

Abstract：Objective To explore the therapeutic effect of intra-aortic balloon pump (IABP) on severe acute left cardiac insufficiency after mitral valve surgery.Methods Nine patients with severe acute left cardiac insufficiency after mitral valve surgery who were treated with IABP in Beijing Anzhen Hospital,Capital Medical University from January 2015 to June 2017 were enrolled.Heart rate(HR),mean arterial pressure (MAP),plasma brain natriuretic peptide (BNP),inotropic drug score (IS),left ventricular ejection fraction (LVEF) and blood lactic acid (Lac) were analyzed before and 24,72 h after IABP.Results All patients were cured and discharged after IABP with average supporting time of (73 ± 8)h(65-92 h).After 24 h of IABP supporting,HR,MAP,BNP,IS,LVEF and Lac were significantly improved compared to those before treatment [(108 ± 6),(99 ± 5) times/min vs (127 ± 9) times/min;(64 ± 5),(74 ± 5) mmHg vs (55 ± 7) mmHg;(806 ± 137),(211 ± 68) ng/L vs (1 961 ±321)ng/L;(18.9 ±2.7),(9.2 ± 1.6) μg/(kg · min) vs (28.2 ± 1.9) μg/(kg · min);(38 ±3)％,(49 ±3)％ vs (27 ±4)％;(3.61 ±0.83),(1.71 ±0.36)mmol/L vs (13.86 ±2.74)mmol/L] (P ＜0.01);HR,MAP,BNP,IS,LVEF and Lac were continuously improved after 72 h of IABP supporting [(99 ± 5) times/min,(74 ±5)mmHg,(211 ±68) ng/L,(9.2 ± 1.6) μg/(kg · min),(49 ± 3)％,(1.71 ± 0.36)mmol/L] (P ＜0.05).A female patient had lower limb skin temperature decreasing at the IABP side,which was ameliorated after increasing perfusion pressure.No adverse reaction was observed in other patients.Conclusion IABP shows a good adjuvant therapeutic effect on severe acute left cardiac insufficiency after mitral valve surgery.
