Effect of prolonged use of antimicrobial agents on postoperative infection and systemic inflammatory response syndrome in patients with radical surgery of hilar cholangiocarcinoma

Abstract:Objective To observe the effect of prolonged use of antimicrobial agents on postoperative infection and systemic inflammatory response syndrome(SIRS) in patients with radical surgery of hilar cholangiocarcinoma.Methods Clinical data of 210 patients who had radical surgery for hilar cholangiocarcinoma in Beijing YouAn Hospital,Capital Medical University from June 2013 to May 2015 were analyzed retrospectively;the patients were divided into control group(102 cases) and observation group(108 cases).All patients were treated with the 3rd generation cephalosporin antibiotics during perioperative period;the control group had 5 d course of antibiotics;the observation group had 8-12 d course of antibiotics.Occurrence of infection 1 week after cessation of drugs and occurrence of SIRS 2,3,5 d after cessation of drugs were analyzed.Results Fifty cases of infection occurred 1 week after cessation of antibiotics,including 34 cases of intra-abdominal infection,6 cases of incision infection,5 cases of pulmonary infection and 10 cases of multiple site infection(abdominal cavity,incision and lung).The total infection rate,abdominal infection rate and multiple site infection rate in observation group were significantly lower than those in control group [15.7% (17/108) vs 37.3% (38/102),9.3% (10/108) vs 23.5% (24/102),1.9% (2/108) vs 7.8% (8/102)] (P <0.05).The incidence of SIRS in observation group was significantly lower than that in control group 2,3,5 d after cessation of antibiotics [13.0% (14/108) vs 27.5% (28/102),8.3% (9/108) vs 32.4% (33/102),3.7% (4/108) vs 25.5% (26/102)] (P < 0.05).Conclusion Appropriate prolonging of the use of antimicrobial agents can reduce incidences of infection and SIRS after radical surgery of hilar cholangiocarcinoma.
