Risk factors of corticospinal tract Wallerian degeneration after intracranial hematoma trepanation and drainage

Abstract:Objective To analyze risk factors of corticospinal tract Wallerian degeneration (CST-WD) after intracranial hematoma trepanation and drainage.Methods Totally 129 patients with basal ganglial hemorrhage were enrolled from January 2012 to December 2016 in Harrison International Peace Hospital;5 patients were withdrawn during observation and 124 patients completed the trial.All patients had intracranial hematoma trepanation and drainage after diagnosis of cerebral hemorrhage by CT.Occurrence rate and risk factors of CST-WD were analyzed.Results CST-WD occurred in 41 patients(33.1%) after operation(CST-WD group);83 patients (66.9%) were grouped as non-CST-WD group.Score of the National Institutes of Health Stroke Scale,amount of bleeding,hemorrhage of internal capsule ratio and the duration time between onset and operation < 12 h ratio in CST-WD group were significantly lower than those in non-CST-WD group [(17 ± 4) scores vs (13 ± 8) scores,(41 ± 8)scores vs (31 ± 11)scores,58.5 % (24/41) vs 31.3 % (26/83),14.6% (6/41) vs 28.9% (24/83)](P < 0.05).Conclusion Duration between onset and operation < 12 h and internal capsule hemorrhage are independent risk factors of CST-WD after intracranial hematoma trepanation and drainage.
