Relation between subclinical hypothyroidism and macrovascular complications in elderly patients with type 2 diabetes mellitus

Abstract:Objective To investigate the relation between subclinical hypothyroidism (SCH)and macrovascular complications in elderly patients with type 2 diabetes mellitus.Methods Clinical data of 168 elderly patients with type 2 diabetes mellitus from May 2014 to June 2015 in the First Affiliated Hospital of the Fourth Military Medical University were retrospectively analyzed;they were divided into control group (without SCH,110 cases) and observation group (complicated with SCH,58 cases).Biochemical indicators,occurrence of macrovascular complications,results of electrocardiogram (ECG) and drug use were recorded.The relation between SCH and macrovascular complications was analyzed by Pearson correlation and logistic regression.Results Age,waist-to-hip ratio,thyroid stimulating hormone(TSH),total cholesterol,fasting C peptide,female ratio in observation group were significantly higher and free thyroxine,glycosylated hemoglobin(HbA1c),ankle-brachial index,left ventricular ejection fraction(LVEF) were significantly lower than those in control group(P < 0.05).Incidences of diabetic foot and abnormal ECG in observation group were significantly higher than those in control group[65.5% (38/58) vs 9.1% (10/110),69.0% (40/58) vs 18.2% (20/110)] (P < 0.05).Pearson correlation analysis showed that TSH was negatively correlated with ankle-brachial index,LVEF,abnormal ECG(r =-0.225,-0.308,-0.325;P < 0.05) and positively correlated with fasting C peptide (r =0.252,P < 0.05).Logistic regression analysis showed that TSH was related to ankle-brachial index and age;LVEF was related to the level of fasting C peptide;abnormal ECG was related to waist-to-hip ratio,sex and the level of HbA1 c (P < 0.05).Conclusion SCH has adverse effects on left ventricular systolic function and lower limb atherosclerosis in elderly patients with type 2 diabetes mellitus;the ankle-branchial index may be a valuable predictor of occurrence of macrovascular complications.
