Clinical value of adenosine for myocardial perfusion imaging in diagnosis of coronary heart disease in elderly women

Abstract：Objective To evaluate the clinical value of adenosine for myocardial perfusion imaging(MPI) in diagnosis of coronary heart disease in elderly women.Methods Clinical data of 65 women over 60 years old with suspected coronary heart disease from February 2008 to August 2012 in Beijing Jishuitan Hospital were retrospectively analyzed.All patients had MPI using adenosine injection and coronary angiography (CAG).The sensitivity and specificity of MPI using adenosine in diagnosis of coronary artery disease were analyzed.Results In 65 cases of suspected coronary heart disease,27 cases (41.5％) had positive results of CAG and 38 cases (58.5％) had negative results of CAG;41 cases (63.1％) had abnormal myocardial perfusion and 24 cases (36.9％) had normal myocardial perfusion;26 cases(40.0％) had positive results of CAG and MPI;23 cases (35.4％) had negative results of CAG and MPI.The sensitivity and specificity of MPI using adenosine in diagnosis of coronary heart disease were 96.3％ and 60.5％,respectively.In 27 patients with positive CAG,there were 16 cases of single vessel lesion,5 cases of double vessel lesions and 6 cases of triple vessel lesions;44 vessels were involved,including 19 anterior descending arteries,14 circumflex arteries and 11 right coronary arteries;36 anterior walls,14 side walls and 20 posterior walls showed low myocardial perfusion.In patients with positive CAG and MPI,there were 37 vessels involved,including 18 anterior descending arteries,10 circumflex arteries and 9 right coronary arteries.The sensitivities of MPI using adenosine in diagnosis of lesions in anterior descending,circumflex and right coronary artery were 94.7％,71.4％ and 81.8％,respectively;the specificities were 60.9％,92.2％ and 79.6％,respectively.Conclusion Adenosine injection for MPI has high sensitivity and specificity in diagnosis of coronary heart disease in elderly women.
