Effect of regional sodium citrate anticoagulation on continuous renal replacement therapy for acute kidney injury after acute aortic dissection operation

Abstract:Objective To explore the effect of regional sodium citrate anticoagulation on continuous renal replacement therapy(CRRT) for acute kidney injury (AKI) after acute aortic dissection operation.Methods A total of 50 patients who had AKI after acute aortic dissection operation from January 2015 to June 2016 in Beijing Anzhen Hospital,Capital Medical University were enrolled and randomly divided into observation group and control group,with 25 cases in each group;2 patients in observation group and 1 patient in control group died during postoperative therapy.The observation group had regional sodium citrate anticoagulation during CRRT;the control group had no anticoagulant intervention.Postoperative related treatment indexs,coagulation function indexes,renal function indexes and related biochemical indicators,death rate and incidence of bleeding events were recorded and compared between groups.Results Service life of filter in observation group was significantly longer,length of Intensive Care Unit stay and total length of hospital stay were significantly shorter than those in control group [(22 ±4)h vs(12 ±4)h,(5.9 ±1.1)d vs(6.9 ± 1.3)d,(16 ±4)d vs(23 ±5)d] (P<0.05).platelet count in observation group was significantly higher than that in control group[(118 ± 25) × 109/L vs (90 ± 25) × 109/L] (P < 0.05).Levels of blood urea nitrogen and serum creatinine before treatment were significantly lower than those after treatment in two groups [observation group:(12 ± 6) mmol/L vs (28 ± 10) mmol/L,(134 ± 20) μmol/L vs (299 ± 30) μmol/L;control group:(16 ± 6) mmol/L vs (28 ± 9) mmol/L,(164 ± 20) μmol/L vs (300 ± 30) μmol/L] (P < 0.05);levels of BUN and SCr in observation group were significantly lower than those in control group after treatment (P < 0.05).The incidence of bleeding events in observation group was significantly lower than that in control group[8.7% (2/23) vs 33.3 % (8/24)] (P < 0.05).Conclusion Regional application of sodium citrate anticoagulation in CRRT treatment can prolong service life of filter and reduce risk of bleeding in patients with AKI after acute aortic dissection operation.
