Effect of nasal non-invasive synchronized intermittent mandatory ventilation on acute exacerbation of chronic obstructive pulmonary disease with respiratory failure

Abstract:Objective To analyze the effect of nasal non-invasive synchronized intermittent mandatory ventilation on acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with respiratory failure.Methods Totally 130 patients with AECOPD complicated with respiratory failure were enrolled from September 2013 to February 2016 in Huangshi Central Hospital of Edong Healthcare Group.The patients were divided into control group and observation group,with 65 cases in each group;the observation group had nasal non-invasive synchronized intermittent mandatory ventilation and the control group had conventional mechanical ventilation.Effective rate,blood gas indexes and complications were analyzed.Results The obvious effective rate in observation group was significantly higher than that in control group[98.5% (64/65) vs 89.2% (58/65)] (P <0.05).Time of mechanical ventilation and length of hospital stay in observation group were significantly shorter than those in control group [(8 ± 3) d vs (11 ± 3) d,(12 ± 4) d vs (18 ± 5) d] (P < 0.05).Values of arterial partial pressure of oxygen(PaO2) after 48 h of ventilation were significantly higher and values of arterial carbon dioxide partial pressure(PaCO2) were significantly lower than those after 12 h of ventilation in both groups [observation group:(69 ± 11) mmHg vs (53 ± 8) mmHg,(65 ± 8) mmHg vs (83 ± 7) mmHg;control group:(61 ± 8) mmHg vs (53 ± 9) mmHg,(77 ± 8) mmHg vs (83 ± 9) mmHg] (P < 0.05).PaO2 after 48 h of ventilation in observation group was significantly higher and PaCO2 was significantly lower than those in control group (P < 0.05).Incidences of hypotension,abdominal distension,nausea,vomiting,dizziness and pulmonary infection had no significant differences between groups (P > 0.05).Conclusion The application of nasal non-invasive synchronized intermittent mandatory ventilation can shorten ventilation time and improve blood gas status in treatment of AECOPD with respiratory failure;both therapeutic effect and safety are satisfactory.
