Clinical effect of off-pump coronary artery bypass grafting on non-ST-segment elevated myocardial infarction

Abstract:Objective To analyze the effect of off-pump coronary artery bypass grafting (OPCABG) on non-ST-segment elevated myocardial infarction(NSTEMI).Methods Totally 300 patients who had OPCABG for NSTEMI from June 2005 to July 2015 in Beijing Anzhen Hospital,Capital Medical University were retrospectively analyzed.Results The preoperative mean left ventricular ejection fraction(LVEF) was (57 ± 5)%;LVEF < 50% was in 75 cases (25.0%);the mean left ventricular end-diastolic diameter was (47 ± 5)mm.Nine patients (3.0%) had single vessel disease;42 patients(14.0%) had double vessel diseases;249 patients(83.0%) had multiple diseases.There were 54 cases(18.0%) of diagonal branch lesions,225 cases(75.0%) of circumflex artery lesions;240 cases(80.0%) of right coronary lesions and 54 cases(18.0%) of old myocardial infarction.Before OPCAB,168 patients (56.0%) were complicated with hypertension;108 patients (36.0%) had diabetes mellitus;24 patients (8.0%) had cerebrovascular diseases.The mean operation time was (4.1 ± 0.5) h;the median graft number was 3(1,5).Total venous grafting was successfully performed in 120 patients(40.0%);the Intensive Care Unit stay time was (45 ± 13)h;the breathing machine time was (45 ± 13)h.Intra-aortic balloon pump was used in 60 patients (20.0%);12 patients were used preoperatively;18 patients were postoperatively supported.Six patients (2.0%) were treated with extracorporeal membrane oxygenation therapy.The postoperative mean LVEF was (57 ± 9) %;major in-hospital complications included re-open operation in 6 cases (2.0%),re-intubation in 12 cases(4.0%),tracheotomy in 2 cases(0.5%),hemodialysis in 3 cases(1.0%) and sternal infection in 3 cases (1.0%).In-hospital death occurred in 6 cases (2.0%).The median follow-up time was 6(1,11) years;the adverse events included major cardiovascular events in 60 patients(20.0%),all cause death in 30 cases(10.0%),re-infraction in 42 cases(14.0%),stroke in 24 cases(8.0%),re-admission in 54 cases (18.0%),re-angina in 75 cases(25.0%) and re-revascularization in 30 cases (10.0%).Conclusion OPCABG has good short-and long-term therapeutic effects on NSTEMI.
