Correlation between level of serum thyroid hormone and severity of chronic obstructive pulmonary disease with acute exacerbation

Abstract:Objective To analyze the correlation between serum thyroid hormone level and severity of disease in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Eighty-four patients with AECOPD in Beijing Jishuitan Hospital from January 2013 to December 2014 were enrolled.The patients were tested serum levels of total triiodothyronine (TT3),total thyroxine (TT4),free triiodothyronine (FT3),free thyroxine (FT4) and thyroid stimulating hormone (TSH) by electrochemiluminescence immunoassay;arterial blood before oxygen inhalation was taken for blood gas analysis;the score of acute physiology and chronic health evaluation scoring system Ⅱ (APACHE Ⅱ) was calculated.Correlations among serum thyroid hormone levels,arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2)and score of APACHE Ⅱ were analyzed via Pearson test.Results Levels of TT3,TT4,FT3 and FT4 were positively correlated with PaO2 (r =0.553,0.430,0.526,0.305;all P < 0.01);Levels of TT3,TT4 and FT4 were negatively correlated with PaCO2 (r =-0.243,-0.326,-0.244;all P < 0.05);Levels of TT3,TT4,FT3 and FT4 were negatively correlated with APACHE Ⅱ score(r =-0.493,-0.519,-0.517,-0.437;all P < 0.01).The level of TSH had no correlation with PaO2,PaCO2,and APACHE Ⅱ score (P > 0.05).Conclusions Serum level of thyroid hormone is correlated with severity of AECOPD.Hypoxia and carbon dioxide retention are related to thyroid hormone decrease.
