Clinical effect of coronary artery bypass grafting on coronary atherosclerotic heart disease in young patients

Abstract:Objective To analyze early clinical effect and medium-long term outcomes of coronary artery bypass grafting(CABG) in young patients with coronary atherosclerotic heart disease(CHD).Methods Clinical data of 602 patients under 50 years old with CHD who had CABG from January 2008 to December 2016 in Beijing Anzhen Hospital,Capital Medical University were retrospectively analyzed,including preoperative basic information,operative notes,perioperative condition,discharge medication and postoperative follow-up data.Results Off-pump CABG was successfully performed in 583 patients and 19 patients had CABG combined with other operations.Left main coronary lesions accounted for 20.8% (125/602);the total number of coronary lesions was (3.1 ± 0.8);the number of coronary anastomosis was (2.9 ± 0.8).Internal mammary artery was used as a bridge vascular in 578 cases(96.0%).Twenty-one patients(3.5%) had emergency operations.Fifty-one patients (8.5 %) had intra-aortic balloon counterpulsation.Perioperative mortality accounted for 0.5% (3/602);causes of death included heart failure [0.3% (2/602)] and infective shock[0.2% (1/602)].Ten patients(16.6%) had re-open operations for bleeding.Fifty-one patients (8.5%) had atrial fibrillation.After discharge,593 patients (97.3 %) had lifetime use of aspirin;589 patients (97.8%) were followed up for 4 (1,7) years;the all-cause of death was 1.3% (8/602);518 patients (86.0%) had no long-term adverse cardiovascular events.Conclusion Early clinical effect and medium-long term outcomes of CABG on CHD in young patients are optimistic.
