Effect of laparoscopic resection and anastomosis on colonic carcinoma complicated with intestinal obstruction

Abstract:Objective To analyze the effect of laparoscopic resection and anastomosis on colonic carcinoma complicated with intestinal obstruction.Methods Totally 82 patients with colonic carcinoma complicated with intestinal obstruction from May 2015 to May 2016 in Xiangyang Central Hospital,Hubei Province were randomly divided into observation group(41 cases) with laparoscopic resection and anastomosis and control group(41 cases) with open abdominal surgery.Operation duration,intraoperative blood loss,length of hospital stay,numbers of lymph nodes dissected,postoperative first exhaust time,satisfaction of operation,recurrence rate,fatality rate,occurrence of incision infection,pulmonary infection,intra-abdominal infection,seroperitoneum and electrolyte disturbance were analyzed.Results The effective rate in observation group was significantly higher than that in control group [36.6% (15/41) vs 19.5% (8/41)] (P < 0.05).Operation duration and length of hospital stay in observation group were significantly shorter than those in control group [(149 ±40)min vs (173 ±44)min,(ll.0 ±2.5)d vs (15.0 ±2.7)d];intraoperative blood loss in observation group was significantly less than those in control group[(105 ± 25) ml vs (138 ± 28) ml];number of lymph nodes dissected in observation group was significantly more than that in control group[(13.2 ± 1.4) cases vs (8.6 ± 1.7) cases];postoperative first exhaust time in observation group was significantly shorter than that in control group [(1.3 ± 0.3) d vs (2.1 ± 0.5) d];the satisfaction score of operation in observation group was significantly higher than that in control group[(8.2 ± 1.2) scores vs (5.1 ± 1.2) scores] (all P < 0.05).The recurrence rate and fatality rate in observation group were significantly lower than those in control group[19.5% (8/41) vs 34.1% (14/41),4.9% (2/41) vs 12.2% (5/41)] (P < 0.05).Incidences of incision infection,pulmonary infection,intra-abdominal infection,intra-abdominal infection,seroperitoneum and electrolyte disturbance in observation group were significantly lower than those in control group[4.9% (2/41) vs 14.6% (6/41),2.4% (1/41) vs 9.8% (4/41),0.0% (0/41) vs 7.3% (3/41),4.9% (2/41) vs 17.1% (7/41),4.9% (2/41) vs 22.0% (9/41)] (P < 0.05).Conclusion Laparoscopie resection and anastomosis is effective in treating colonic carcinoma complicated with intestinal obstruction.
