Clinical effect and relevant factors of glucocorticoid in treatment of severe ulcerative colitis

Abstract:Objective To analyze related factors of clinical effect of glucocorticoid on severe ulcerative colitis.Methods From January 2015 to January 2016 in Xian XD Group Hospital,100 patients with severe ulcerative colitis who were treated with aminosalicylic acid plus prednisone were enrolled as observation group;100 patients who were treated with aminosalicylic acid alone were enrolled as control group.Therapeutic effect was observed after 12 weeks of treatment.Related clinical data and mucosa manifestations under enteroscope were analyzed.Results Clinical remission rate in observation group was significantly higher than that in control group [76.0% (76/100) vs 60.0% (60/100)] (P =0.015).According to therapeutic effect,patients in observation group were divided into complete remission group (30 cases),partial remission group (46 cases) and ineffective group(24 cases).Proportions of patients with disease cause > 5 years,smoking and drinking,diarrhea ≥ 10 times/d on admission,severe attack history,hemoglobin < 90 g/L,defecation ≥6 times on the 3rd day of treatment and bloody stool on the 3rd day of treatment had significant differences among 3 groups [36.7% (11/30),76.1% (35/46) vs 87.5% (21/24);26.7% (8/30),43.5% (20/46) vs 79.2% (19/24);20.0% (6/30),37.0% (17/46) vs 54.2% (13/24);10.0% (3/30),28.3% (13/46) vs 41.7% (10/24);30.0% (9/30),34.8% (16/46) vs 50.0% (12/24);10.0% (3/30),23.9% (11/46) vs 41.7% (10/24);30.0% (9/30),67.4% (31/46) vs 70.8% (17/24)] (P < 0.05).Rate of severe lesion under enteroscope in ineffective group was significantly higher than that in complete remission group and partial remission group[95.8% (23/24) vs 36.7% (11/30),52.2% (24/46)] (P < 0.05);rate of widely mucosa injury in ineffective group was significantly higher than that in complete remission group [50.0% (12/24) vs 23.3% (7/30)] (P < 0.05);rate of deep ulcer in ineffective group was significantly higher than that in complete remission group and partial remission group[25.0% (6/24) vs 3.3% (1/30),10.9% (5/46)] (P <0.05).Conclusions Glucocorticoid is effective in treating severe ulcerative colitis.Disease cause course > 5 years,smoking and drinking,diarrhea ≥ 10 times/d on admission,severe attack history,hemoglobin < 90 g/L,defecation ≥6 times on the 3rd day of treatment and bloody stool on the 3rd day of treatment may be related to the curative effect of glucocorticoid.
