Clinical effect and adverse reaction of nasal mask and face mask connected with non-invasive positive pressure ventilation in treatment of respiratory failure in patients with acute myocardial infarction

Abstract:Objective To analyze clinical effect and adverse reaction of nasal mask and face mask connected with non-invasive positive pressure ventilation(NIPPV)in treatment of respiratory failure in patients with acute myocardial infarction.Methods Fifty AMI patients with respiratory failure from January 2013 to January 2015 in Beijing Tongren Hospital,Capital Medical University were completely randomly divided into nasal mask group(n =25) and face mask group(n =25) in the treatment with NIPPV.Arterial blood gas indexes and adverse reactions(electrolyte disturbance and metabolic alkalosis) were analyzed.Results PH value and arterial partial pressure of oxygen significantly increased and arterial partial pressure of carbon dioxide decreased after treatment 1 week in both groups (P < 0.05);the differences between groups were not significant (P > 0.05).Level of sodium ions significantly increased and level of potassium ions decreased after treatment 1 week in both groups (P < 0.05).After treatment 1 week,level of sodium ions in nasal mask group was significantly higher and level of potassium ions was lower than those in face mask group [(138 ± 4)mmol/L vs (148 ± 6)mmol/L,(3.8 ± 0.6) mmol/L vs (3.0 ± 0.5) mmol/L] (P < 0.05).Incidence of metabolic alkalosis significantly increased after treatment 1 week in both groups;the incidence in face mask group was higher than that in nasal group [68.0% (17/25) vs 40.0% (10/25)] (P < 0.05).Incidence of adverse reactions in nasal mask group was significantly lower than that in face mask group [12.0% (3/25) vs 40.0% (10/25)] (P < 0.05).Conclusions Nasal mask and face mask are effective in treatment of acute myocardial infarction complicated with respiratory failure,positive pressure mechanical ventilation can improve blood gas and aeration condition in patients.But using face mask is more likely to lead to metabolic alkalosis.
