Clinical efficacies and prognoses of different therapies in treatment of coronary atherosclerotic heart disease in octogenarian patients

Abstract:Objective To analyze clinical efficacies and prognoses of different therapies treating coronary atherosclerotic heart disease(CHD) in octogenarian patients(over 80 years).Methods Totally 855 patients aged over 80 years with CHD diagnosed by coronary angiography from May 2005 to January 2016 in Beijing Anzhen Hospital,Capital Medical University were enrolled;564 cases had percutaneous coronary intervention (PCI group),143 cases had coronary artery bypass grafting(CABG group),148 cases had drug treatment(drug group).Left ventricular ejection fraction(LVEF),left ventricular end-diastolic dimension(LVEDD),coronary angiography results,Barthel score,Seattle angina questionnaire (SAQ) score and cardiovascular death were analyzed.Results LVEF,LVEDD and Barthel score had no significant differences among 3 groups at admission (P > 0.05).The Barthel score in CABG group was significantly lower than that in PCI group and drug group at discharge[(61.2 ±3.4)scores vs (88.6±0.9),(88.1 ±4.7)scores] (P <0.05).Ratios of left main coronary lesion,calcified lesion,> 20 mm lesion,small vessel lesion(lumen diameter < 3 mm)and the number of lesions had no significant differences among groups(P > 0.05).One year after discharge,Barthel score had no significant difference among groups(P > 0.05);scores of physical limitation,anginal frequency,treatment satisfaction and disease perception in SAQ in PCI group and CABG group were significantly higher than those in drug group(P < 0.05),the score of anginal stability had no significant difference among groups(P > 0.05).Cardiovascular death rates in PCI group and CABG group were significantly lower than the death rate in drug group[4.4% (25/564),4.2% (6/143) vs 14.2% (21/148)] (P < 0.05).Conclusion Revascularization therapy is more effective than drug therapy for octogenarian patients with CHD.
