Effect of different antiplatelet therapies on platelet function in acute coronary syndrome patients with clopidogrel resistance

Abstract:Objective To analyze antiplatelet therapies with ticagrelor and cilostazol on platelet aggregation rate in acute coronary syndrome (ACS) patients with clopidogrel resistance.Methods A total of 86 ACS patients with clopidogrel resistance (platelet aggregation rate > 50%) were retrospectively enrolled from November 2007 to March 2016 in Beijing Anzhen Hospital,Capital Medical University.The patients used to have antiplatelet therapy with aspirin and clopidogrel;64 cases of them had ticagrelor replacing clopidogrel;22 cases were added cilostazol.Platelet aggregation rate was analyzed.Results The platelet aggregation rate in ticagrelor group was significantly lower than that in cilostazol group[(24 ± 11) % vs (26 ± 28) %] (P < 0.01).The decline rate and the standard-reaching rate of platelet aggregation rate in ticagrelor group were significantly higher than those in cilostazol group [67.12%(50.41%,75.47%) vs28.15%(8.41%,50.51%),96.9% (62/64) vs63.6%(14/22)](P< 0.01).Conclusion Clopidogrel resistance patients with high bleeding risk can use cilostazol in combination with aspirin and clopidogrel;patients with low bleeding risk can use aspirin and ticagrelor.
