Characteristics of coronary artery lesion, cardiac structure and function in acute coronary syndrome complicated with rheumatoid arthritis between different genders

Abstract:Objective To investigate characteristics of coronary artery lesion,cardiac structure and function in acute coronary syndrome(ACS) complicated with rheumatoid arthritis(RA) between different genders.Methods Fifty-two ACS patients complicated with RA were enrolled from April 2012 to January 2016 in Beijing Anzhen Hospital,Capital Medical University;62 ACS patients without RA (age and sex matched) were enrolled as a control group.General information,laboratory indexes,cardiovascular risk factors,SYNTAX score,echocardiogram parameters and serum level of brain natriuretic peptide (BNP) were analyzed.Results Serum level of highdensity lipoprotein cholesterol(HDL-C) in male RA group was significantly lower and high sensitivity C-reactive protein(hs-CRP) was higher than those in female RA group [(0.89 ± 0.20) mmol/L vs (0.99 ± 0.21) mmol/L;24.55 (5.08,33.57) mg/L vs 3.63 (1.60,17.94) mg/L] (P < 0.05).Proportions of smoking and drinking in male RA group were significantly higher than those in female RA group[57.1% (12/21) vs 12.9% (4/31),19.0% (4/21) vs 0.0% (0/31)] (P < 0.05).Left ventricular end-diastolic diameter(LVEDD),left ventricular endsystolic dimension(LVESD) and serum level of BNP in male RA group were significantly higher and left ventricular ejection fraction(LVEF) was lower than those in female RA group [(50 ± 8)mm vs (46 ± 4)mm,(37 ± 5)mm vs (30 ± 4) mm,208.35 (99.50,627.30) ng/L vs 172.00 (73.00,332.70) ng/L,(53 ± 3) % vs (61 ± 11) %] (P <0.01 or P <0.05).LVEDD,LVESD and BNP in RA group were significantly higher and LVEF,mitral early diastolic peak flow velocity (E)/late diastolic peak flow velocity (A) were lower than those in control group [(48 ±6)mm vs (47 ±12)mm,(34 ±7)mm vs (33 ±6)mm,191.60(92.51,451.15)ng/L vs 176.52(85.77,398.60)ng/L,(55 ±9)% vs (61 ± 17)%,0.72(0.64,1.17) vs 0.82(0.62,1.23)] (P <0.05).Conclusion Severity of coronary artery lesion in patients with ACS complicated with RA has no obvious difference between genders;damages on left ventricular function and cardiac structure in male are more severe.
