Effect of home oxygen therapy and breathing exercise in combination with salmeterol fluticasone powder inhalation on stable chronic obstructive pulmonary disease

Abstract:Objective To explore the effect of home oxygen therapy and breathing exercise in combination with salmeterol and fluticasone powder inhalation on patients with stable chronic obstructive pulmonary disease (COPD).Methods Totally 110 patients with COPD from January 2015 to January 2016 in Chongqing Red Cross Hospital(People's Hospital of Jiangbei District) were randomly divided into observation group and control group,with 55 cases in each group.The control group was treated with salmeterol and fluticasone powder inhalation when the disease condition was stable after hospitalization;the observation group was treated with inhalation combined with home oxygen therapy and breathing exercise for 6 months.Pulmonary function,blood-gas analysis,blood routine indexes and BODE index were determined.St.George's Respiratory Questionnaire and Family Assessment Device(FAD) (including subjective symptom,life quality,life self-care ability and medical expense) were performed to evaluate the disease improvement.The times of COPD acute exacerbation were recorded.Results After treatment,percentage of predicted forced expiratory volume in one second (FEV1%),percentage of predicted forced vital capacity (FVC%),arterial partial pressure of oxygen (PaO2)and hemoglobin were significantly higher;arterial partial pressure of carbon dioxide (PaCO2) were significantly lower than those before treatment in both groups[observation group:(52 ±6)% vs(41±5)%,(71 ±6)% vs(62±6)%,(65±5)mmHg vs(57±4)mmHg,(165 ± 14)g/L vs(128 ± 11)g/L,(54 ± 6) mmHg vs(63 ±6) mmHg;control group:(46 ±5)% vs (41 ±6)%,(68 ±7)% vs(62 ±5)%,(61 ±5) mmHg vs(56 ±4) mmHg,(158 ± 15) g/L vs(126 ± 14) g/L,(56 ± 5) mmHg vs (64 ± 6) mmHg] (P < 0.05);FEV1%,FVC%,PaO2 in observation group were significantly higher and PaCO2 were significantly lower than those in control group (P < 0.05).BODE index and disease improvement according to St.George's Respiratory Questionnaire and FAD in observation group were significantly better than those in control group (P < 0.05).COPD acute exacerbation cases and times in observation group during 6 months of treatment were lower than those in control group.Conclusion Long-term home oxygen therapy and breathing exercise combined with salmeterol and fluticasone powder inhalation treating stable COPD can improve lung function and life quality.
