Clinical effect of captopril on viral myocarditis in children

Abstract:Objective To explore the therapeutic effect of captopril on viral myocarditis in children.Methods Totally 88 children with viral myocarditis in Intergrated Traditional Chinese and Western Medicine Hospital of Dazhou,Sichuan Province from August 2014 to February 2016 were randomly divided into control group (44 cases) and observation group(44 cases).The control group had routine comprehensive therapy with ribavirin and astragalus injection;the observation group was added captopril adjuvant therapy.Total effective rate,electrocardiography results,myocardial enzymes and cardiac function indexes were analyzed.Results The total effective rate in observation group was significantly higher than that in control group [97.7% (43/44) vs 84.1% (37/44)] (x2 =4.95,P =0.02).Incidences of ST-T change and arrhythmia after treatment in observation group were significantly lower than those in control group[27.3% (12/44) vs 63.6% (28/44),25.0% (11/44) vs 56.8% (25/44)] (P < 0.05).Serum levels of creatine kinase,creatine kinase isoenzyme,cardiac troponin Ⅰ,aspartate aminotransferase and lactate dehydrogenase in observation group were significantly lower than those in control group [(115±26)U/Lvs (138±28)U/L,(13 ±4)U/L vs (20±5)U/L,(0.126 ± 0.071) μg/L vs (0.228± 0.160) μg/L,(23 ± 5) U/L vs (32 ± 5) U/L,(50 ± 17) U/L vs (78 ± 21) U/L] (P <0.05).Left ventricular ejection fraction,left ventricular fraction shortening and diastolic peak flow velocity late(PFVA) in observation group were significantly higher than those in control group [(68 ± 10) % vs (60 ± 10) %,(36 ± 8) % vs (29 ±7)%,(0.91 ±0.10)m/s vs (0.79 ±0.11)m/s] (P <0.05);early diastolic peak flow(PFVE) and PFVA/PFVE ratio in observation group were significantly lower than those in control group [(0.47 ± 0.09) m/s vs (0.57±0.13)m/s,(0.52±0.09) vs (0.68±0.11)](P<0.05).Conclusion Captopriladjunctive therapy for children with viral is effective;it can correct anomalous electrocardiology,improve heart function and lower serum levels of various myocardial enzymes.
