Correlations among high density lipoprotein cholesterol, severity of coronary disease and vascular endothelial function in coronary atherosclerotic heart disease

Abstract：Objective To investigate the correlations among high density lipoprotein cholesterol (HDL-C),severity of coronary disease and vascular endothelial function in patients with coronary atherosclerotic heart disease(CHD).Methods From April 2014 to October 2015,146 cases of CHD and 97 non-CHD patients were enrolled in Zhengzhou University People's Hospital.General clinical data,blood lipid level and vascular endothelial function were analyzed.Influence factors of vascular endothelial function were analyzed using Logistic regression model.The correlation between HDL-C and vascular endothelial function in CHD patients was analyzed by Pearson analysis.CHD patients were divided into single-vessel disease group (37 cases),double-vessel disease group (48 cases) and triple-vessel disease group (61 cases);the difference of vascular endothelial function was analyzed among 3 groups.Results The level of HDL-C and reactive hyperemia peripheral index(RHI) in CHD patients were significantly lower than those in non-CHD patients [(0.99 ± 0.21) mmol/L vs (1.08 ± 0.24) mmol/L,(1.6 ± 0.3) vs (1.8 ± 0.5)] (all P ＜ 0.05).The level of HDL-C was significantly correlated with vascular endothelial function in 243 patients(odds ratio =3.171,P =0.032).The level of HDL-C was significantly correlated with vascular endothelial function in CHD patients (odds ratio =5.132,P =0.043),and HDL-C and RHI were positive correlated(r =0.470,P ＜ 0.001).Blood pressure was correlated with RHI in non-CHD patients (odds ratio =4.611,P =0.005).In CHD patients,RHI decreased with severity of coronary disease [(1.52 ± 0.23),(1.44 ±0.19),(1.40 ± 0.19)] (P =0.025);there was a significant difference of RHI between single-vessel disease group and triple-vessel disease group (P =0.007).Conclusions HDL-C is a protective factor of vascular endothelial function in CHD patients.Vascular endothelial function can be a predictive factor of the severity of coronary disease.
